Epidemiology Data Request Form

Health Department data sets are available through a data use agreement. You will need to complete an agreement template and send proof (email or letter) from your institution’s institutional review board that states they have approved your research project.

Once we receive the template and approve it, you and our Deputy Commissioner will sign the final agreement. The process can take a month or more.
